If you're planning on bringing a pet to Maui, here are some helpful resources and contacts to make the process easier:

  • Maui Humane Society Direct Release Timeline Worksheet is a useful summary of the entire process, and will help you stay on track while focusing on the current step.
  • Neighbor Island Inspection Permit Checklist contains all the information you need to obtain the Neighbor Island Inspection Permit. These documents are all that the state requires to issue the permit.
  • Dog & Cat Import Form. Use the link provided to fill out the form online, then print it once it's completed.
  • Letter Requesting Direct Release Template you can use for your request letter, but any letter that includes your pet's microchip number will do.
  • MHS Airport Inspection Questionnaire can be filled out online. Once you submit the form, you'll receive payment instructions from MHS.

The Animal Quarantine Station requires submission of all paperwork and fees at least 30 days prior to your arrival, so that it arrives before you depart. There are two separate fees that must be submitted for the permit to be generated - one fee to the state, and another fee to MHS. Refer to the MHS Fee Schedule Below for further information.

Once quarantine approves your entry, they will send a ARC (Airport Release Card) to MHS. Once they have the ARC, they will contact you for further instructions on who will meet you at the airport, where you'll meet, and how to obtain your health certificate.

Fees:

  • First Time Arrival for Family Pets: $350 first pet and $100 for additional pet/s.
  • First Time Arrival for Service Animals: $200 first pet and $100 for additional pet/s.
  • Repeat arrivals for all: $175 per pet and $100 for each additional pet/s.
  • After-hours fee for arrivals after 5pm, weekends & Holidays: $100 additional fee.
  • Flight change fee $50.
  • Flight cancellation fee $100.
